Veilshrouded Wraith of the Forgotten Ocean

A ghostly specter wreathed in ethereal, flowing mist, this entity emerges from the depths of forgotten seas, where drowned memories whisper through the tides. Possessing the ability to weave illusions of dread, it invokes the fears of its enemies, bringing forth visions of their darkest moments. Its presence is both haunting and mesmerizing, often luring unsuspecting sailors and adventurers alike to their doom. Innate Spellcasting. The wraith's spellcasting ability is Charisma (spell save DC 17). It can innately cast the following spells, requiring no material components:

  • At will: minor illusion, silent image
  • 3/day each: fear, darkness, phantom lesser restoration
  • 1/day each: confusion, phantasmal killer

Ethereal Sight. The wraith can see 60 feet into the Ethereal Plane when it is on the Material Plane, and vice versa.

Illusory Manifestation. As a bonus action, the wraith can create an illusory duplicate of itself that lasts until the end of its next turn. All creatures within 30 feet of the wraith must succeed on a DC 17 Wisdom saving throw or become frightened until the end of their next turn. A creature that succeeds on this saving throw is immune to this ability for 24 hours.

Horrifying Visage. Any creature that starts its turn within 30 feet of the wraith and can see it must succeed on a DC 17 Wisdom saving throw or be frightened until the end of its next turn. A creature that succeeds on this saving throw is immune to this effect for 24 hours.

Actions

Multiattack. The wraith makes two attacks: one with its Corrupting Touch and one with its Dreadful Wail.

Corrupting Touch. Melee Spell Attack: +9 to hit, reach 5 ft., one creature. Hit: 24 (4d10 + 2) necrotic damage, and the target must succeed on a DC 17 Constitution saving throw or have its maximum hit points reduced by an amount equal to the damage taken. This reduction lasts until the target finishes a long rest.

Dreadful Wail (Recharge 5-6). The wraith unleashes a harrowing wail. Each creature within 60 feet that can hear it must make a DC 17 Wisdom saving throw. On a failed save, a creature takes 36 (8d8) psychic damage and is incapacitated until the end of its next turn. On a successful save, a creature takes half as much damage and is not incapacitated.
