Mist-Wreathed Warden of the Forgotten Depths

This ethereal guardian manifests as a swirling mass of gray mist, its form partially hidden within tendrils of fog that seem to reach out menacingly. Glowing eyes peer from the depths of the vapor, instilling both dread and awe in those who encounter it. This creature is a remnant of ancient guardianship, bound to protect sacred places long forgotten. With each movement, the mists swirl chaotically, creating an aura that muddles perceptions and confounds senses. Those who approach may find it increasingly difficult to discern what is real as their intentions are challenged by the creature's powerful enchantments.

Mist Form. The creature can move through creatures and objects as if they were difficult terrain. It takes 5 (1d10) force damage if it ends its turn inside an object.

Spectral Presence. As a bonus action, the creature can choose one creature it can see within 60 feet of it. That creature must succeed on a DC 15 Wisdom saving throw or become confused for 1 minute. While confused, the creature cannot tell friend from foe and must roll a d6 at the start of each of its turns to determine its action (1-2: attack a random creature; 3-4: take no action; 5-6: move in a random direction).

Mist Veil (Recharge 5-6). The creature envelops itself in an impenetrable mist for 1 minute, becoming invisible and granting disadvantage on attacks made against it. While in this state, it can still use its spectral presence ability.

Actions
Multiattack. The creature makes two attacks with its Miasmic Touch.
Miasmic Touch. Melee Spell Attack: +6 to hit, reach 5 ft., one target. Hit: 22 (4d10) necrotic damage, and the target must succeed on a DC 15 Constitution saving throw or have its hit point maximum reduced by an amount equal to the damage taken until it finishes a long rest.

Legendary Actions
The creature can take 3 legendary actions, choosing from the options below. Only one legendary action option can be used at a time and only at the end of another creature's turn. The creature regains spent legendary actions at the start of its turn.

  • Mist Step. The creature moves up to its speed without provoking opportunity attacks.
  • Dimensional Drift (Cost 2 Actions). The creature teleports up to 30 feet to an unoccupied space it can see.
  • Enveloping Fog (Cost 3 Actions). The creature creates a 20-foot radius fog cloud centered on itself for 1 minute. The area is heavily obscured, and creatures within the fog have disadvantage on Wisdom (Perception) checks that rely on sight. Creatures inside the fog must also succeed on a DC 15 Constitution saving throw at the start of their turn or take 10 (3d6) psychic damage.
