Elixir of Temporal Reversal

This shimmering liquid swirls with iridescent colors that shift like moving shadows. When you hold the vial up to the light, fleeting images of events long past dance within, teasing the boundaries of memory and time itself. Once per long rest, upon consuming this potion, you can reverse a single event that occurred within the last 6 seconds during combat or a similar high-stakes situation.
When activated, the world blurs for all but you as the scene unfolds in reverse. Any actions taken by your allies or enemies during that span, including damage dealt, spells cast, and movement, are undone. Note that any creatures affected must still remember the actions taken; they do not lose their conscious awareness of what happened, merely their physical impact.
However, the lingering effects of this manipulation come with a twist. After using the potion, roll a d10. On a roll of 1–3, your perception of time is altered for the next hour. You gain disadvantage on all initiative rolls, as moments feel distorted, and you struggle to react swiftly to incoming threats.
The potion does not allow any alteration to the result of catastrophic failures (such as death). If used in response to an imminent critical hit or doom, those moments cannot be undone, but a near miss might still alter the outcome. The intent behind the potion is one of caution and strategy rather than mere recklessness, encouraging clever use over brash decisions.
The vial itself cannot be duplicated, and those who possess it may find themselves observed by others who covet its power, or even face unforeseen consequences from meddling with the natural flow of time.
