This shimmering, ethereal entity drifts above the surface of sorrowful waters, drawn from the deepest despair of lost souls. Its form is amorphous, swirling in shades of blue and gray, with flickering accents that resemble starlight. Gossamer tendrils stretch from its body, reaching out with a chilling touch that harbors both sorrow and an unsettling allure. As it glides silently through the misty air, it seeks to latch onto the emotions of those nearby, weaving illusions that prey on their unresolved grief and longing. Ethereal Touch. A creature that touches or is touched by this entity must succeed on a DC 16 Wisdom saving throw or become affected by the confusion spell for 1 minute. The creature may repeat the saving throw at the end of each of its turns, ending the effect on itself on a success.
Illusory Grief. As a bonus action, the entity can create a 30-foot radius sphere of murky water filled with sorrowful whispers. Creatures within the sphere must succeed on a DC 16 Wisdom saving throw or take 10 (3d6) psychic damage and be frightened until the end of their next turn. Creatures that succeed on the saving throw take half damage and are not frightened.
Divine Reflection. When targeted by an attack or a spell, the entity can use its reaction to become incorporeal until the start of its next turn, causing the attack or spell to miss and dissipate into ambient sorrow. On a successful miss, the attacker must make a DC 16 Wisdom saving throw or be stunned until the end of their next turn.
Innate Spellcasting. The entity's innate spellcasting ability is Charisma (spell save DC 16). It can innately cast the following spells, requiring no material components:
- At will: Minor Illusion, Fear
- 3/day each: Suggestion, Phantasmal Killer
- 1/day each: Greater Invisibility, Confusion

Touch. Melee Spell Attack: +8 to hit, reach 5 ft., one creature. Hit: 18 (4d8) psychic damage. If the target is a creature, it must succeed on a DC 16 Wisdom saving throw or become incapacitated until the end of its next turn.
Sorrowful Burst (Recharge 5-6). The entity releases a wave of sorrowful energy in a 30-foot radius centered on itself. Each creature in that area must make a DC 16 Constitution saving throw, taking 28 (8d6) psychic damage on a failed save, or half as much damage on a successful one. Additionally, affected creatures experience disadvantage on Wisdom saving throws for 1 minute. A creature can repeat the saving throw at the end of each of its turns, ending the effect on itself on a success.
