Setting: The Whispering Grotto
The Whispering Grotto is an eerie cave system located on the outskirts of the village of Eldenthorn, known for its hauntingly beautiful stalactites and massive underground chambers. These caves are rumored to be inhabited by lost spirits whose whispers constantly echo between the stone walls, creating an unsettling ambiance. Many villagers refuse to enter, claiming that the whispered words can drive a person mad. Ancient carvings litter the walls, telling stories of an ancient civilization that revered the whispers as omens, but this civilization mysteriously vanished.
Recently, the whispers have grown louder and more frantic, plunging the nearby village into a dark panic. An adventuring party is called upon to uncover the source of this disturbance, hoping to restore peace to Eldenthorn.

Goal
The party must venture into the Whispering Grotto to confront the Wraith of Forgotten Memories, stop the spate of madness affecting Eldenthorn, and free the tormented souls caught within the cavern. Along the way, the players must solve traps and puzzles, navigate treacherous terrain, and face both the Wraith's illusions and its minions.
The Journey Into the Grotto
As the players approach the cave entrance, a cold wind beckons them inside, carrying the echoes of whispered words. Flickering lanterns light their path, but shadows dance in and out of sight. The atmosphere is thick with tension, and players must make a DC 14 Wisdom saving throw or feel a wave of despair wash over them, imposing disadvantage on their next ability check.
First Chamber: The Hall of Echoes
Upon entering the first chamber, the players encounter a large vat of shimmering silver liquid situated in the center. The walls are covered in ancient runes that pulse, reflecting the whispers. To uncover the secret of the vat, players must decipher the runes, which is a DC 16 Intelligence (Arcana) check. If they succeed, they discover that the liquid will reveal the truths hidden by the whispers when poured into the hands of a willing participant.
However, as they investigate, shadowy figures emerge from the walls—lost companions trapped between realms. These spirits appear as twisted reflections of themselves, with disheveled appearances and hollow eyes. The Lost Companions will attack players, attempting to drag them into the shadows. Combat is unavoidable unless players can convince one of the spirits of their goodwill (DC 16 Charisma check).
The Puzzle of Truth
Once the players have dealt with the Lost Companions, they can proceed to solve the Puzzle of Truth, which involves pouring the silver liquid over a designated area to reveal clues. Players must arrange stone tablets found scattered within this chamber using a puzzle “gear” mechanic that matches the symbols found on the ancient runes. Each stone tablet represents a key memory that players must align with certain positions on a central stone circle to unlock the hidden door leading deeper into the grotto.
Players must remember that each incorrect arrangement triggers a psychic backlash, causing them to roll a DC 15 Constitution saving throw. On failure, the affected player will take 3d6 psychic damage and suffer from the Frightened condition until the end of their next turn.
Confrontation: The Veil of Whispers
Upon completing the Puzzle of Truth, the party enters a larger chamber known as the Veil of Whispers, filled with mist and ethereal lights that twist and coil like living entities. The Wraith of Forgotten Memories stands at its epicenter, manipulating shadows around it.
Illusions and Combat
The Wraith calls upon its powers, creating horrifying shadows that mirror the deepest fears of the players. These illusions can feel real—ghostly images of loved ones in peril, manifestations of their own failures, or dark predictions of their futures. Players must succeed on a DC 16 Wisdom saving throw or be paralyzed in fear.
Combat begins with the Wraith attacking from the shadows, capable of using both necrotic damage and psychic attacks. As it does so, it may use its powers to manipulate the Forgotten Companions, causing them to rise against the players once more. Players should be wary as they battle; the Wraith draws strength from fear, gaining +1 to attack rolls for each player affected by its illusions.
To defeat the Wraith, players must locate the medallion of the ancient civilization—the only item capable of holding the Wraith at bay. It is hidden beneath a pile of obsidian rock in the cavern's center, providing an additional challenge as players navigate through the fray to retrieve it.
Resolution: The Spirit's Release
Once the Wraith is defeated, the illusions dissipate, leaving behind only the echoes of the whispers. The Lost Companions appear, appearing calmer now as their forms gradually stabilize. One of them, a former knight named Varyn, thanks the party for releasing them. Liora materializes, joining the scene as the spirits begin to cross over peacefully into rest.
As a reward for their bravery, Liora grants the party a temporary boon—advantage on Wisdom saving throws for the next 24 hours. The spirits express their gratitude, revealing that the village will be forever indebted to the adventurers for their noble actions.
Eldenthorn returns to normalcy, the whispers now a soft, gentle hum as the party exits the grotto, leaving behind only memories of their brave journey amid shadows, whispers, and unexpected truths. The village celebrates their return as heroes, but the echoes of the abyss remain—a reminder of the encounters left unfaced and the past that still clings to the shadows.
