Sunken Spirits. When a creature that is not undead comes within 30 feet of this entity, it must succeed on a DC 16 Wisdom saving throw or be subjected to the "Haunting Melodies" effect. On a failure, the creature becomes incapacitated until the start of its next turn, reliving the memories of the sunken souls.

Actions

Multiattack. The creature makes two Shadow Touch attacks.

Shadow Touch. Melee Weapon Attack: +7 to hit, reach 5 ft., one target. Hit: 27 (5d10) necrotic damage. The target must succeed on a DC 16 Constitution saving throw or have its maximum hit points reduced by an amount equal to the necrotic damage taken. This reduction lasts until the target finishes a long rest.

Spectral Grasp (Recharge 5–6). The entity extends a shadowy tendril that reaches out to a creature within 30 feet. The target must make a DC 16 Dexterity saving throw. On a failed save, the target takes 45 (10d8) necrotic damage and is pulled 15 feet closer to the entity. On a successful save, the target takes half damage and is not pulled.

Rejuvenating Depths (1/Day). The creature can call upon the energies of the sunken depths to regain 50 hit points. If the entity is in a body of water, it regains an additional 10 hit points.

Reactions

Evasive Shroud. When an attack misses, the entity can use its reaction to become partially incorporeal until the end of its next turn, giving it a +2 bonus to AC until then.
